What will you do? (Key Responsibilities): Your primary mission will be to manage and implement the program’s nutritional strategy, ensuring each patient receives the necessary education and support for their well-being. * Intervention and Education: Design and implement personalized nutritional education plans for patients and caregivers, focused on managing their specific pathology. * Clinical Management: Conduct technical follow-up and data monitoring jointly with the assigned case management team. * Patient Support: Provide guidance on the correct use of APME and report adverse events (pharmacovigilance) to ensure patient safety. * Team Coordination: Technically coordinate and advise field staff (assistants/case managers) and collaborate closely with treating physicians to validate nutritional proposals. * Strategy and Quality: Contribute to meeting program indicators, ensuring data quality and proposing continuous improvement actions. * Impact: Contribute to the holistic well-being of the population served, promoting self-care and treatment adherence. Job Requirements: * Education: Degree in Nutrition and Dietetics (with valid professional license). * Experience: Minimum 1 year in clinical nutrition, outpatient consultation, or health promotion and prevention (P&P) programs. Prior experience in Patient Support Programs or APME management is desirable. * Knowledge: Solid expertise in nutritional assessment, APME, and current health regulations. * Key Competencies: o Excellent assertive communication skills and empathy (in interacting with patients and families). o Leadership and ability to collaborate effectively within interdisciplinary teams. o Results orientation and organizational skills for indicator management. o Proficiency in office tools (Excel, CRM).
