




**Overview** We are seeking a proactive, organized, and empathetic **On\-Call Coordinator** to support our operations during weekends and after\-hours coverage. This role is critical to ensuring continuity of care, timely caregiver scheduling, and clear communication with patients, caregivers, and internal teams. The On\-Call Coordinator acts as a key point of contact during non\-standard business hours, ensuring that issues are resolved promptly and care services continue without disruption. **Key Responsibilities** **Care Coordination \& Scheduling** * Schedule and assign caregivers to patients in homes or facilities based on care needs and staff availability * Provide on\-call coverage during weekends and designated after\-hours shifts * Respond promptly to calls, messages, and urgent scheduling needs * Ensure all visits are confirmed and accurately recorded in systems such as **HHAexchange and/or Salesforce** **Communication \& Documentation** * Communicate clearly with caregivers, patients, and families regarding assignments, changes, or service issues * Document all communications and actions taken in the system * Track caregiver availability, acceptance of assignments, and eligibility * Escalate clinical concerns or patient condition changes to the clinical team **Operational \& HR Support** * Initiate intake processes by creating or updating patient profiles * Coordinate with HR and recruitment teams regarding staffing needs * Act as a liaison with contract case managers and schedulers * Ensure compliance with internal policies and applicable federal, state, and local regulations **Additional Responsibilities** * Respond to voicemails and emails within required response times * Support staff satisfaction through effective communication and problem resolution * Maintain strict confidentiality of patient, caregiver, and company information **Required Skills \& Qualifications** * Bilingual: **English and Spanish (spoken and written)** * Strong communication and customer service skills * High attention to detail and strong organizational abilities * Experience in care coordination, scheduling, or home health operations preferred * Proficiency in Microsoft Office (Excel, Outlook, Word) * Experience with **HHAexchange, Salesforce, or similar platforms** is a plus * Ability to multitask and remain calm under pressure * Willingness to work weekends, on\-call shifts, and after\-hours coverage **Why Join Evo?** As an **On\-Call Coordinator**, you play a vital role in ensuring patients receive timely, reliable care when it matters most.
