Job Summary: Provides assistance to physicians and other healthcare providers in patient assessment, planning, and care delivery, while monitoring and managing stabilized chronic conditions and educating patients on healthcare management. Educating and advising patients on healthcare management **About the Role** Provides assistance and support to physicians and other healthcare providers in patient assessment, planning, and care delivery. Assesses physical and psychosocial health status by collecting health data. Identifies problems based on interpretation of findings. Evaluates the need for immediate nursing intervention, consultation, and referral to other members of the healthcare team. Collaborates with physicians in developing policies, procedures, and protocols for managing specific health issues. Monitors and manages care for stabilized chronic conditions. Educates, advises, and guides patients on healthcare management. **Key Responsibilities** * Preparation of digital and/or manual clinical records for all patients treated at Convatec Medical Care, including corresponding progress notes, in compliance with established standards. * During induction or periodic follow-up consultations, establish the treatment plan according to the patient’s clinical condition, following Convatec Medical Care’s clinical practice guidelines and protocols for high-, medium-, and low-complexity patients. * Archive patient follow-up photographs in digital folders (organized by insurer, patient, and date). * Respond to user requests for advisory support via phone calls. * Forecast and report treatment plans for new patients enrolled in the outpatient or home-based wound clinic program. * Prepare patient reports and forms as required. * Submit monthly reports on patients assessed at home and participate in audits. * Evaluate and supervise adherence to clinic hygiene standards during cleaning and disinfection of the examination room. * Evaluate and supervise adherence to infection control and prevention measures during patient care, per clinic standards. * Coordinate the requisition and delivery of supplies to PAD nurses and maintain a complete record of these activities. **Requirements** * Licensed Registered Nurse. * Location: Cartagena. * Valid C1 driver’s license or license application in process (mandatory). * Experience in wound and ostomy clinics. * In-depth knowledge of Convatec products and services. Excellent verbal and written communication skills. * Exceptional customer service skills and ability to multitask under pressure. * Attention to detail, time management. * In-depth knowledge of wound and ostomy patient care protocols. * Technical skills and ability to monitor patient treatments and identify complications. * Demonstrated ability to work effectively in a team. * Preferred: specialization in auditing with providers or insurers or in education. **Ready to join us?** At Convatec, we are revolutionizing trusted medical solutions to improve the lives we touch.
