Coordination of care and guidance on maternal health Fertility clinic seeks a Clinical Follow-Up Assistant. Title: Social Worker Availability: One weekend every 15 days Responsibilities: Support the coordinator of first-time pregnant women's processes. Conduct telephonic or digital follow-up of pregnant patients according to established protocols. Verify attendance at prenatal check-ups, tests, and procedures. Identify warning signs or symptoms and promptly refer to the appropriate healthcare professional. Record follow-up activities clearly and timely in the clinical record or institutional system. Schedule, confirm, and follow up on medical appointments, check-ups, and tests. Coordinate care with prepaid medicine services, insurance providers, or agreements. Manage rescheduling and ensure continuity of care. Support the medical and nursing team in non-medical activities. Guide patients on care pathways, preparation for tests, and adherence to check-ups. Support documentation management related to authorizations, consents, and records. Provide basic maternal health education per institutional guidelines (without issuing diagnoses). Resolve operational queries and guide patients within the scope of responsibilities. Maintain constant communication among the patient, treating physician, and institution.
